Prioritising prevention & improving future health outcomes through tackling health inequalities
As a system, we want to prioritise supporting our population to remain as independent and healthy as possible, whilst also providing effective, timely and accessible treatment and care when required, from early years through to the end of life.
We know that the wider determinants of health – such as housing, employment and education – are closely related to a person’s health. We also know that certain groups and communities are more likely to experience poorer health outcomes than others. That’s why, as a system, we are committed to helping address some of these underlying causes and ensure that we work towards reducing health inequalities.
We have identified three key areas that we need to focus on in order to prioritise prevention and improve future health outcomes locally. They are:
- Reducing health inequalities
- Prioritising prevention and wider determinants to protect the health and wellbeing of people and communities
- Enabling the best start in life for children and young people
