Planned Care

We’re aiming to help people stay well and offer more care out of hospital. This will enable our acute hospitals to focus on caring for people who are seriously ill or who need specialist treatment from experts.

 

What do we want to do?

  • Provide better patient care and reduce demand in planned care.
  • Offer more support in the community, so that more people could benefit from services such as rehabilitation and ongoing care at, or nearer to, their home.
  • Only bring people to hospital if that is the best place for them.
  • Help people to come home from hospital sooner with the right support to continue their recovery.

 

 

What have we done so far?

We’re focusing on seven key areas: demand management, musculoskeletal, pathway redesign, performance, planned care contracts, policy development and resources.

We regularly engage with the public to find out patients’ views on their experiences, from seeing their GP to going into hospital for treatment.
