COVENTRY & WARWICKSHIRE INTEGRATED CARE SYSTEM – NURSING PATIENT SAFETY LEARNING EVENT

Thank you to all NHS colleagues, healthcare professionals, community providers, primary care teams, charities, and trainers who participated in our recent patient safety event in Coventry.
The session reinforced that patient safety is a shared responsibility and provided valuable opportunities for collaboration and learning.
Event Focus
The event centred on the Patient Safety Incident Response Framework (PSIRF), the NHS’s approach to incident response that prioritises learning and improvement over blame.
Key Highlights
The morning opened with a warm welcome from Sharon Binyon, Deputy Chief Medical Officer at Coventry and Warwickshire ICB.
- George Eliot Hospital: Spoke about their co-designed patient safety incident leaflet, a Health Services Journal (HSJ) award finalist.
- South Warwickshire University Foundation Trust: Shared outcomes from a PSIRF-led pressure ulcer project, demonstrating reductions in incidents and harm.
- University Hospital Coventry and Warwickshire: Discussed the impact of specialist training for over 50 staff.
- Learning from the Lives and Deaths of People with a Learning Disability (LeDeR): Emphasised the lifesaving importance of asking early about a patient’s health passport.
- West Midlands Ambulance Service: Outlined their cultural transformation from compliance to compassion, improving staff morale and perceptions.
- Coventry and Warwickshire ICB: Provided updates on Primary Care’s progress with PSIRF and the Infection Prevention Team shared resources about hand hygiene.
- Health Innovation West Midlands: Encouraged reflection on the maturity of PSIRF implementation plans.
The event was expertly compared by Lorna Jones, CWICB Deputy Director Nursing for Quality & Patient Safety, who encouraged networking and knowledge sharing among attendees over lunch.

This was the third learning event hosted by the Coventry and Warwickshire Care Board reinforcing that patient safety remains central to the work of our Integrated Care System despite a demanding and evolving environment.
