HOW WE’RE CREATING THE FUTURE OF HEALTH AND CARE IN COVENTRY AND WARWICKSHIRE

The NHS in Coventry and Warwickshire is launching a new campaign that showcases how health and care organisations are joining forces to improve healthcare outcomes for local people.
‘The Future of Health and Care in Coventry and Warwickshire’ will shine a spotlight on how real people are benefiting from the integrated care approach and are leading happier, healthier lives.
The ‘integrated’ approach to health and care has seen GPs, acute hospitals, mental health services, councils and health care commissioning services join up to streamline the way patients access care and improve their overall experiences. In turn this helps to maximise the health and care resources available to local communities in Coventry and Warwickshire.
Throughout the campaign, real patients will be stepping forward to share their experiences about how they have benefited from the new integrated approach whether that has meant they have received quick diagnosis and access to treatment or have had dedicated support to better manage long-term conditions, reducing the need for treatment altogether.
Some of the stories include how Fiona experienced quicker diagnosis and access to treatment for colorectal cancer and how Sam’s life changed thanks to a new integrated self-management approach to musculoskeletal care.
Phil Johns, Chief Executive Officer of Coventry and Warwickshire Integrated Care Board, said:
“For a long-time health and care organisations have worked independently to one another despite all prioritising the same goal – to improve the health of their local communities.
“By taking an integrated approach to healthcare, we’re putting the patients’ needs at the centre of everything we do which means that the care patients receive will be more coordinated and continuous, leading to better outcomes and experiences.
“Over the past two years we have been making progress as an Integrated Care System to link up all our healthcare providers across Coventry and Warwickshire to focus on exactly what different communities across the region need; but we are not where we need to be yet, and we are continuing to seek new ways to integrate.
“We’ve been bringing care closer to communities, removing the need for our patients to visit acute hospital sites, as seen through the development of our Primary Care Gynaecological Service (PCGS) which moved gynaecological services from acute hospitals to community settings, leading to a reduction in waiting times for patients.
“Through new technologies we’ve been able to proactively investigate health data to identify ways to proactively support people at risk of becoming ill, as demonstrated through a new programme that was introduced in Sowe Valley which saw a personalised and preventative model of care developed to support patients with Type 2 diabetes.
“Moving forward we’re continuing to introduce new ways of working through integration and innovation to pave the way for the future of health and care in Coventry and Warwickshire.”
