APOLLO FRAILTY HUB: HELPING PEOPLE LIVING WITH FRAILTY STAY HEALTHIER AT HOME

The situation
As we get older, some people become frail and may be living with several long-term health conditions. This can make everyday life more difficult and increase the chance of falls, emergency hospital visits and unplanned admissions.
The Apollo Frailty Hub was created to identify people who may need extra support earlier, so they can receive personalised care before their health deteriorates.
Using the Health Data and Intelligence (HDI) Platform, healthcare teams across six GP practices in Warwickshire North were able to identify people who could benefit most from the programme and offer them the right support at the right time.
What we did
The HDI Platform helped bring together information from different health services to identify patients who may benefit from extra support.
People invited to the Frailty Hub received a detailed assessment from a team of healthcare professionals. Depending on their needs, this could include:
- A review of their overall health and wellbeing
- Falls prevention advice and physiotherapy support
- A review of their medicines by a clinical pharmacist
- Personalised care planning with a GP
- Help to access local community and voluntary services
The healthcare team also met regularly to discuss patients with more complex needs and make sure everyone involved in their care was working together.
The impact
Since April 2025, the Frailty Hub has supported 1,140 patients across Warwickshire North.
The programme has helped:
- Reduce A&E attendances for one in three patients who were evaluated
- Review more than 8,600 medicines to improve safety and reduce unnecessary medication
- Prevent an estimated 39 avoidable adverse events
- Improve joined-up care between GPs, pharmacists, physiotherapists and community services
- Help people receive more personalised support to stay well and independent for longer
The programme has also delivered an estimated £177,000 in savings during the first year, with projected savings of more than £209,000 over 12 months, helping to make better use of NHS resources.
Next steps
The Apollo Frailty Hub will continue to identify people who may benefit from earlier support and provide personalised care that helps them stay healthier for longer.
The team is also continuing to use insights from the HDI Platform to improve care, reduce avoidable hospital visits and ensure people living with frailty receive the support they need from the right professionals at the right time.
