Coventry & Warwickshire Integrated Care Record

Ever wondered why, when you visit your GP or hospital, they can’t see all your health and care information? And why you find yourself having to answer the same old questions over and over – about things like the medicines you take, the treatment you’ve had, and whether you have any allergies?

The simple answer is they all use different computer systems to record your details. And because these systems aren’t connected, the health and care organisations looking after you can’t see the information each other holds on you.

But that’s now changing with the introduction of the Coventry and Warwickshire Integrated Care Record (ICR).

Our experience of working with COVID-19 has shown how important it is for the health and care professionals caring for a person to be able to see their information without delay when needed. The ICR will make a joined-up approach to health and care much more possible.


What is an Integrated Care Record (ICR)?

An Integrated Care Record (ICR) is a way of bringing together the various electronic records of a person’s care. It takes information directly from existing systems used by health and social care organisations and presents it in a structured, easy-to-read format for health and care professionals.

The ICR will provide health and care professionals with a more joined-up view of people’s care and treatment across all care settings. 

It has been developed in response to the feedback from many of you over a number of years, telling us that you only want to tell your story once when receiving care from any health or social care organisation across Coventry and Warwickshire.


What data will be available and who will be able to see it?

The first phase of the ICR will allow health and care professionals to view:

  • Your GP practice medical record
  • Secondary care and mental health data

Having access to this information will help them provide you with the best care as quickly as possible without having to make phone calls or wait for details to be forwarded by fax or email.

Initially we will bring together information from GPs and provide a view of this to hospitals, including Emergency Departments. And we will add community and social care, ambulance and NHS 111 data.  

We will continue to develop the ICR, allowing professionals across more health and social care settings to see information needed to support your care. We will also help services understand and find the best ways to meet people’s care needs.

There are strict rules about how we use your information and, as part of this work, we will ensure it is managed and viewed appropriately and in line with all legal requirements. These include UK Data Protection legislation (DPA 2018 and superseding legislation). All access will also be subject to audit.


Which organisations are involved with the Coventry & Warwickshire ICR?

The following organisations are involved in the ICR programme:

  • GP practices within Coventry and Warwickshire through their membership of the clinical commissioning groups 
  • Coventry and Warwickshire Partnership NHS Trust
  • University Hospitals Coventry and Warwickshire NHS Trust
  • George Eliot Hospital NHS Trust
  • South Warwickshire NHS Foundation Trust
  • Coventry City Council 
  • Warwickshire County Council 
  • West Midlands Ambulance Service University NHS Foundation Trust

Health and care organisations in the neighbouring areas of Birmingham and Solihull, and Herefordshire and Worcestershire, will be able to view your information for the purpose of giving you direct care should it be necessary.


What do I need to do?

You don’t need to do anything. For anyone aged 18 or over who is registered with a GP in Coventry and Warwickshire, Birmingham (excluding west Birmingham) and Solihull, or Herefordshire and Worcestershire, the change is taking place automatically as we now switch on the ICR across the organisations taking part in the programme. We’ll update the list of organisations over time to reflect any changes that may take place.

Your GP practice can choose not to take part in the ICR programme. If that is the case for your practice, the information in your GP records will not be available for other health and care professionals to view through the ICR.  Please check with your GP practice if you would like to know whether it is taking part.


The benefits to you include:

  • Not having to repeat your details every time you have a different episode of care
  • Better and potentially faster treatment due to health and care professionals having access to better information
  • Reduction in potential medication errors through clinicians having access to your medication history (past and present)
  • More effective treatment should you need care for COVID-19 due to fast access to information about any pre-existing conditions and your medications


What if I do not want my records to be available through the ICR?

You have a right to object at any time. We don’t recommend this, as information that could be vital when you need health or social care support – for instance, during a visit to a hospital Emergency Department – might not be immediately to hand as a result.
Objecting will mean the services giving you care will be unable to view your records from other services.

However, the decision is entirely yours. If you do want to object, you can find out how on our Right to Object page.

Please note, the ICR is different to anything you might have said no to before. So, if you don’t want your records to be available to view through it, you’ll need to object.
