how to embed personalised care: practical first steps for organisations, managers and individuals

For organisations - first steps to embedding a personalised approach

1. Use the Coventry & Warwickshire Personalisation Maturity Framework to benchmark where you, your team, service and organisation are now. 

2. Identify a Personalised Care Champion or Ambassador for your organisation and in Directorates.

2. Identify training gaps, and review the training available to Coventry & Warwickshire colleagues, teams and organisations.

3. In Primary Care, check those additional roles mandated for personalised care training. The Health Education England First and Advanced Practitioner Roadmaps highlight the additional roles requiring training in personalised care.

5. Use the Coventry & Warwickshire Personalisation Maturity Framework to develop an action plan to embed a personalised approach to patient care.

6. Ensure that there is a long-term programme in place to build shared decision making capability within the workforce, including making shared decision making training a part of every new member of staff’s induction.

What good looks like

  1. All clinical team members demonstrably practice shared decision making.
  2. There is a Personalised Care Champion or Ambassador in your organisation and in Directorates.
  3. Staff are supported to attend workshops and webinars on how to apply personalised care in their day-to-day practice.
  4. Staff are able to access support from the C&W Personalised Care programme and access website resources and toolkits.
  5. All clinical team members have an awareness of the importance of shared decision making and all six components of personalised care.
  6. All members of clinical teams have been trained in shared decision making and simplified communication techniques (such as 'teach back'), and are able to check whether complex information has been explained in a way that makes sense to people.
  7. There is a long-term programme in place to build shared decision making capability within the service workforce.
  8. Shared decision making and the six components of personalised care form part of every new member of staff’s induction.
  9. Shared decision making is recognised as an ongoing CPD need for clinicians.

For managers - first steps to developing a personalised care approach and skills in your team:

1. Develop your own skills in leadership for personalised care.

2. Use the Coventry & Warwickshire Personalisation Maturity Framework to benchmark where your team or service is now. 

3. Support your staff to undertake personalised care training including e-Learning. In addition to the free training resources available, can you use CPD or other funding to support further, in-depth training? (Contact us for advice and information.)

4. Include personalised care in goal setting objectives for individuals.

5. Include personalised care in team meetings and team action plans. Use the Coventry & Warwickshire Personalisation Maturity Framework to develop an action plan to embed a personalised approach in your service or pathway.

7. Plan an ‘It’s OK to Ask’ campaign, to help your patients get the best outcomes from personalisation.

8. Attend managers’ personalised care workshops and webinars, and arrange regular reflection practice workshops for your team to support their knowledge, skills, and application of personalised care.

9. Provide your team with access to support tools and resources from the C&W personalisation team.

We recommend reviewing existing practice in your team. Some teams have recruited a facilitator to help them reflect on their existing practices of shared decision making in their services and how they can strengthen these, including coaching for managers. There are a number of different options to consider, from two day Shared Decision Making team sessions to a series of shorter or longer sessions over an 8–12-week period.
