Medicines Optimisation in Care Homes
We provide specialised support for care providers working in care homes to improve the quality and safety of medicines use for care home residents.
Our role involves collaborating with our ICB and ICS system partners to promote the safe and effective use of medicines within care homes.
We work in partnership with GPs, care home staff, health professionals and multi-agencies to address medicines management related issues or queries.
We can provide support, advice, and best practice guidance to care homes in relation to Medicines Optimisation. If you require any support or have any medicines optimisation issues related to care homes, please contact us via email at cwicb.mot@nhs.net.
