Developing new ways of working

The Programme has also allowed us to trial new innovations that make a real difference for patients. We intercepted ambulance calls for frail patients and dealt with them directly, leading to a 20% reduction in over 80s needing to go to hospital by ambulance.

We increased physiotherapy care in the community to reduce the need for patients to come into hospital for an appointment and transformed our neurophysiology pathway, so that patients were able to have a virtual appointment first and be assessed by a specialist physiotherapist, rather than travel into the hospital for tests.

GPs were also willing to provide new patient appointments and procedures which have traditionally been delivered in a hospital setting.

These successful workstreams will now be picked up by other services, improving the patient experience and letting us make the best use of our resources.
